Homeward Bound Horses and Hounds appreciates your patience and understanding throughout this application process. Please understand our goal and responsibility is to make sure the animals in our care are placed in only the best homes.

By signing this document, you give Homeward Bound Horses and Hounds permission to contact your vet, Landlord & references during the application process. We reserve the right to check any local, state and federal do not adopt lists and to conduct background checks to ensure there are no animal related offenses. Homeward Bound Horses and Hounds has to the right to refuse an adoption for ANY reason. This application remains the property of Homeward Bound.

I certify that the above information is true. I understand and agree that this adoption placement is conducted solely for the purpose of providing long term care for the adopted animal. Should a representative of Homeward Bound ask to see the animal, access will be granted. If it is determined that the animal is not properly cared for, the animal will be reclaimed by Homeward Bound. Further, I agree to pay any reasonable expenses, including legal fees that may be incurred by Homeward Bound in the event that the adopted animal placed in my custody is reclaimed by Homeward Bound because of neglect or abuse of said adopted animal.

I am fully aware of the risks of adopting an animal with an unknown history. I understand that some animals may come from abusive homes. I hereby for myself, my heirs, my personal representatives agree to the following:

1. I assume all risks associated with the adoption of an animal from Homeward Bound Horses and Hounds Animal Rescue, Inc. including but not limited to serious bodily injury and / or death.
2. I hereby waive, release, discharge, hold harmless and promise to indemnify and not bring suit against Homeward Bound Horses and Hounds Animal Rescue, Inc., its Directors, Officers, Volunteers, and all other agents, and attorneys for any of the referenced parties. I also hereby waive, release, discharge, hold harmless and promise to indemnify and not bring suit against any other parties acting on behalf of Homeward Bound from any and all rights and claims which I have or which may hereafter accrue to meand from any and all damages which may be sustained by me directly or indirectly in connection with, or arising out of, my exposure to such animals.
